myVaccine Record

You can now use the myColorado app to easily access your digital COVID-19 vaccine record.

  1. If you haven't already, download the myColorado app™ and set up an account using your Colorado driver license or state ID.

  2. Once your account is set up, click the myVaccine Record button on the Wallet screen.

  3. Review the myVaccine information and click "Continue."

  4. Confirm your First Name, Last Name, Date of Birth and Gender.

  5. Enter your phone number or email address.

  6. Click "Submit."

Your digital COVID-19 vaccination record card should appear in 10 seconds or less! You will then have the ability to download it to your device if you wish.

COVID-19 vaccination(s) display in the same format as the paper CDC card and can be shown anywhere that proof of vaccine is required.

Accessing your record

How soon after receiving a COVID-19 vaccine can I access myVaccine record?
It will take a minimum of 24 hours before your vaccinations will reflect on your digital vaccination record card.

Can I access my children’s vaccine record?
Unfortunately, no. You are only able to access your own personal record.

Can I access other immunization records besides my COVID-19 vaccination information?
No. You are only able to access your COVID-19 vaccination record.

Why can’t I access my vaccination record via myVaccine Record?
While most providers in Colorado report vaccinations to the Colorado Immunization Information System (CIIS), federal providers do not. If you received your COVID-19 vaccine from a federal provider, please contact the health care professional or clinic where you received your vaccine and ask them to report your record to CIIS.

Please note: The contact information you enter on the myColorado app must match the contact information associated with your record in CIIS. If you have questions about your immunization record, please fill out this form to request assistance.

I received an error message stating “We didn’t find an exact match”. What should I do?
The Department of Public Health and Environment (CDPHE) has set up a help desk to assist you. Please fill out this form to request assistance. The CDPHE help desk is extremely busy right now, so please expect a turnaround of 5-7 business days.

I received an error message stating “More than one immunization record was found”. What should I do?

My contact information was recently updated in CIIS. When can I expect to be able to access my record?
It will take a minimum of 24 hours for your updated contact information to reflect on your account.

Why does myColorado use multi-factor authentication (MFA) for logging in?
Multi-factor authentication (MFA) helps to keep your account more secure by requiring account credentials along with additional information (e.g. a verification code sent to your phone.) This makes it harder for someone who is not authorize to access your account.

I received a COVID-19 vaccine(s) in Colorado but I can’t access myVaccine Record in the myColorado App for one or more of the following reasons:

  • I don’t have a Colorado ID or license

  • I don’t have a smartphone

  • I don’t have data/WiFi/internet connectivity

Are there other ways for me to obtain a copy of my COVID-19 vaccine record?
Yes, in addition to the paper CDC COVID-19 Vaccination Record Card you were given when you received your COVID-19 vaccine(s) and myVaccine Record, you can obtain a copy of your COVID-19 vaccine record via:

  • https://vaccines.colorado.gov

    • Anyone who received vaccines in Colorado can obtain a copy of their vaccine records that exist in the Colorado Immunization Information System (CIIS).

  • Your primary care provider

    • You can contact your primary care provider and request a copy of your vaccine record.


What if CIIS doesn’t have my email address or phone number?
Please contact cdphe.ciis@state.co.us to request that your email address and/or phone number is added to your account. It will take at least 24 hours for your account to be updated into myColorado.

I was vaccinated in another state, how do I report this to Colorado so I can access myVaccine Record in the myColorado app?

f you are a Colorado resident and have a Colorado driver license or state-issued ID you can call your provider in the state you received your vaccine and ask them to report your vaccination record via email to CIIS at cdphe.ciis@state.co.us. As soon as your vaccination information has been reported to CIIS you will be able to pull your own myVaccine Record in the myColorado app.

If you do not have a Colorado driver license or state issued ID you will not be able to use the myVaccine Record feature at this time.

Using your vaccine record

How do I update myVaccine Record after I receive my second dose?
Open your myVaccine Record and click the green refresh button at the top of the screen. Enter the same contact information you entered when you accessed your existing record.

What if my digital vaccine record is incorrect?
Please fill out this form to request assistance. The CDPHE help desk is extremely busy right now, so please expect a turnaround of 5-7 business days. We apologize for any inconvenience this causes.

I don’t want my immunization record card displayed anymore. Can I delete it?
Yes. Click the trash can icon in the upper righthand corner and follow the prompts.

Will my immunization information remain private?
Yes! Use of the myColorado app to retrieve vaccination records is voluntary on an opt-in basis. No data will be accessed or shared except that which is requested by a myColorado user who has been authenticated through facial recognition. A retrieved vaccination record is only visible to the myColorado user who downloaded it, and the data is not shared with anyone else in the retrieval process.

Is this a vaccine passport?
No. The digital CDC COVID-19 Vaccination Record Card is an way to view your COVID-19 vaccine information. It is a convenient digital version of your paper vaccination record card that draws your immunization information directly from Colorado’s Immunization Information System (CIIS).

myColorado™ General FAQs

Why won’t the app accept my selfie?

The most common facial recognition issues are:

  • Newly-issued licenses can take up to 4-7 business days to become available in myColorado. Please allow a minimum of 24-48 hours for your new driver license or state-issued ID to appear in the app.

  • When taking your selfie Poor lighting, out-of-focus selfie or cluttered background.

Tips: When taking your selfie, avoid poor lighting and cluttered backgrounds. Take your selfie against a clean, white background in a well-lit room, making sure your face is in focus and evenly lit without shadows.

  • If you try three times and the selfie authentication does not work, we have found that sometimes deleting the app and reinstalling will work. You will be able to use the account you already created.

Note: If your driver license or ID photo is more than five years old, you may need to update your license/ID at the DMV as the older image quality may be difficult to match and verify identity.

Will my information remain private?
Yes! Use of the myColorado app to retrieve vaccination records is voluntary on an opt-in basis. No data will be accessed or shared except that which is requested by a myColorado user who has been authenticated through facial recognition. Why can’t I find the myColorado app in the Google Play Store?
The Google Play store only displays apps that are compatible with your phone. If you search and can’t find the app, you may need to upgrade your phone or operating system.

Troubleshooting

Where did my downloaded vaccine record go (Android users)?

Some LG and Motorola phones don’t come with an app that lets you open downloaded PDF documents. If you have downloaded your vaccine record but cannot locate the file on your phone, you may need to install a PDF reader app on your phone to access the file. Several free apps are available on the Google Play store.

Where did my downloaded vaccine record go (IOS users)?

If you are using an iOS device simply download the PDF and access via your Files app. Click Browse -> iCloud Drive -> myColoradoicloudcontainer folder.

If you are not able to locate your PDF download in your Files app or you receive a “Sign into iCloud” error please check your iOS settings.

First, confirm that you are signed into your iCloud account. Once you are signed in make sure that your iOS Wallet app is enabled. To make sure your Wallet is enabled go to your Settings -> Apple ID -> iCloud -> Wallet. Click the slider so that it is green or in the “on” position. To disable your Wallet, click the slider again until it turns grey or in the “off” position.
